Live Phish Vol. 16 is a live album by Vermont-based jam band Phish released on October 29, 2002. It was recorded live at Thomas and Mack Center located on the campus of the University of Nevada, Las Vegas, in Paradise, Nevada on Halloween night, 1998.

It marks the fourth Halloween show in which Phish dressed up in a "musical costume" by performing an album from another artist in its entirety. On this night, the band unveiled a complete performance of the Velvet Underground's Loaded sandwiched in between two sets of Phish's own music.

This performance is the only Halloween performance that sees Phish straying away from the album's original format, instead embarking on several extended improvisational jams throughout the set. Two of the album's songs - "Lonesome Cowboy Bill" and "Sweet Jane" - had already been performed by Phish in concert. The group later admitted that they could not fully decide on a choice for the 1998 Halloween album so they decided to go with an album that they already knew for the most part.

Fan reaction was mixed as many were not familiar with the album, although "Rock & Roll" would later become a Phish concert staple.

Disc four is a bonus disc recorded the night before as part of the band's 15th Anniversary celebration.

Though it is not the longest concert in the series, this is the longest volume in the Live Phish Series with the bonus disc bringing the total length to just under five hours.

Live Phish Volume 16 è un album dal vivo dei Phish, pubblicato il 29 ottobre 2002 (in contemporanea ai Volumi 13, 14 e 15 della serie Live Phish) dalla Elektra Records. Il disco riporta l'esibizione del gruppo al Thomas and Mack Center di Las Vegas, nella notte di Halloween del 1998. È il quarto di 6 spettacoli di Halloween in cui i Phish hanno eseguito dal vivo in un unico concerto un intero album di un altro artista: nel caso della performance in questione, il gruppo svelò la sera stessa che avrebbe eseguito durante il secondo set l'album dei Velvet Underground Loaded. Come spesso accade per i lunghissimi concerti della band, anche questo fu diviso in successive uscite (o "set"). Questa è l'unica performance di Halloween nella quale i Phish si discostarono dalla scaletta del disco originale, eseguendo lunghe improvvisazione (soprattutto nei brani Rock & Roll e Lonesome Cowboy Bill). L'esibizione della notte di Halloween è contenuta nei primi 3 dischi; il 4 riporta invece una parte del concerto eseguito dalla band sempre al Thomas and Mack Center la sera precedente (30 ottobre 1998), ed è composto di pezzi usualmente eseguiti dai Phish. Benché il concerto della notte di Halloween del 1998 non sia il più lungo dei Phish, con l'aggiunta del disco 4 il Volume 16 raggiunge quasi le 5 ore di musica e risulta il disco più lungo della serie Live Phish (il più breve è invece il Vol. 3).
